Welcome to the fourth module for VITAL: Relational Health.
This module focuses on how you, the busy primary care provider in clinical practice, can use surveillance questions, screening tools and conversational approaches to surveil and monitor the relational health of your patients and their caregivers.

LEARNING OBJECTIVES


By the end of this module participants will be able to:
1. List at least two approaches for surveilling and monitoring relational health in a busy practice.
2. List at least two questions you could ask your patients to start a conversation about relational health.
