Welcome to the fifth module for VITAL: Relational Health.
This module will review the current screening and assessment tools for relational health. However, given that there are limited short, evidence-based tools, this module will give practical suggestions for how to translate this work for the busy primary care practice.

LEARNING OBJECTIVES


By the end of this module participants will be able to:
1. List at least two evidence-based screening or assessment tools for relational health.
2. List at least three questions you could ask your patients to start a conversation about relational health.
